The Healthy and Well in Hyde Park Collaborative is working together
for a healthier, more equitable community.

Historical Background

Hyde Park residents have come together to address health disparities, including limited access to primary care services, preventative care, and health education, that challenge our neighborhood. A spirited effort, led by the Healthy and Well Collaborative, is underway to establish a Health and Wellness Center in Hyde Park, one that would be similar to health centers in other Boston neighborhoods but would be the first within our neighborhood  a very underserved part of the city in terms of access to healthcare.

We seek a responsive healthcare center that can begin to provide services tailored to the diverse needs of our community, assisting those lacking healthcare options and dealing with maternal health, chronic health, and mental health issues.

Why Hyde Park Needs a Health Center

Data from the Boston Public Health Commission reveals significant health
disparities in our neighborhood.

Community Health Centers

Hyde Park is one of only two Boston neighborhoods without a community health center.

Heart Disease Mortality Rate

Per 100,000 residents — significantly higher than Boston's overall rate of 92.1.

Adults with Diabetes

Compared to 20% across the rest of Boston Hyde Park has a significantly higher rate.

Asthma ED Visits per 10K

Hyde Park residents visit the ED for asthma at higher rates than the rest of Boston (69.6).

Mission Statement

Our mission as the Healthy and Well in Hyde Park Collaborative (HWC), as an inclusive and representative group of community stakeholders, is to work to establish a dedicated community health center in Hyde Park to provide all Hyde Park residents with equitable, preventative medical services and wellness resources.

We will accomplish this through:

In collaboration with neighbors and allies at our center, we will create a community that exudes health, wellness, longevity, and vibrancy.
